Block Heads Issues

I will create a Head Plugin like Java Edition, and i have 3 Problems:

1. My Entitys teleporting up after restart

2. I cant place entity on wall (Its buggyyy)

3. (Main Problem) When i place the entity/block with other directions it's very buggy.

What can i do to fix this three things?
