There shouldn't be a single plugin to transfer players with a sign. There should be a plugin that handles all dynamic signs, and a plugin that provides the variables for showing online players... Anyway, this is semi-possible using Hormones. You just have to have a plugin that runs a command when you click a sign (e.g. TapToDo), and set the command of clicking a sign as "/organ OtherOrgan" so that the players will be transferred to a server of the OtherOrgan. Hormones provides a more pragmatic solution of transferring than specifying the server IP every time
